Hospice RN at The Judge Group

Location: Middleborough Center, MA
Description: Our client is currently seeking a Hospice RN Case Manager in the Southeast Massachusetts area!

This is a direct hire opportunity offering competitive compensation and benefits!


HOURS:

Mon- Fri, 8:30a-5p

On-call rotation - 1 overnight/week and once every 5th weekend

The Hospice RN is responsible for providing nurse and hospice case management to hospice beneficiaries and families, implementing nursing interventions and coordinating the beneficiary/family interdisciplinary plan of care. 


This job will have the following responsibilities:
•Participate in Interdisciplinary Team (IDT), manage and coordinate the beneficiary and family/caregiver Plan of Care from referral to discharge. 
•Facilitate communication about changes in the beneficiary's status among IDT members and the beneficiary's attending physician.
 •Plan, coordinate, and deliver direct nursing care to beneficiaries and families according to the beneficiary's need. 
•Visit each beneficiary at least every 14 days in accordance with regulatory requirements. 
•Participate in coordination of care activities with families, physicians, and host nursing facility partners, such as: care coordination meetings, at-risk meetings, information presentations, or meet and greets. 
•Perform an initial and comprehensive assessment of the beneficiary/family and develops a Plan of Care and ensures the care plan is up to date and reviewed at every team meeting. 
•Notify physician and/or appropriate others of significant findings or changes in the beneficiary's physical or mental condition. 
•Consult with physicians regarding the care of the beneficiary as changes in the beneficiary's POC occur and at least every 14 days. 
•Assess beneficiary needs for necessary therapies (PT, OT, and ST) and consult to assist in quality of life and comfort care. 
•Complete Hospice Aide schedule each week and provide SNF/caregivers with copies of the schedule. Communicate all schedule changes as soon as they are made. 
•Supervise and document Hospice Aide Plans of Care and document supervisory visits.


Qualifications & Requirements:
  • RN with valid MA RN License 
  • Previous Hospice or Palliative Care experience
  • Valid drivers license and reliable car
  • Must be fully vaccinated for COVID-19
  • Previous management/supervisory experience preferred.
