They are seeking a dynamic and reliable Community Health Worker to join their growing team- They have a 16-bed acute in-patient unit, Case Management, Partial Hospitalization Program, and Adult Community Clinical Services- Under supervision, you will provide physical and emotional care to patients in inpatient settings Community Health Worker Responsibilities:- Provides programmatic direct care services daily and assisting clients in learning the skills of daily living- Maintain patient records which may include daily reports, round sheets- Responsible for completion of housekeeping tasks- Accompany patients to various appointments or programs or treatment areas Community Health Worker Qualifications:- 1+ years of related experience- Mental Health or psychiatric experience is required- Compassionate and patient There are no costs for candidates utilizing our services as our clients pay for all related hiring fees.
